Our facility is accredited by the American Board of Certification in Orthotics, Prosthetics, and Pedorthics. Therapeutic Shoes (aka diabetic shoes) and/or inserts qualify for coverage under the Therapeutic Shoes for Individuals with Diabetes benefit (Social Security Act §1861(s)(12)). This is a brief overview of the process necessary to obtain diabetic shoes/inserts that qualify for coverage under your Medicare and/or insurance benefits.

Please understand that you are responsible for paying the upfront costs for your products and services before a claim is submitted to your insurance for reimbursement; Burkhartzmeyer Shoes does not accept assignments from Medicare or other insurance agencies. After your payment to Burkhartzmeyer Shoes, we will then submit your claim to Medicare and/or your insurance for the approved items. Typically, Medicare will cover up to 80% of approved items, including; one pair (1) of diabetic shoes and three pairs (3) of diabetic inserts (if applicable) per calendar year. Your reimbursement will come directly from Medicare, and any remaining amount may or may not be covered by your supplemental insurance.

The foot examination for diabetic shoes and/or inserts can take anywhere from 30-60 minutes, so please call ahead and schedule an appointment so we may have time allocated to serve you. Our Certified Fitter of Therapeutic Shoes (CFts) and/or Certified Pedorthist (CPed) will examine your feet, discuss your needs, and fit your shoes/inserts during this time. Please read carefully the list below to ensure you have the proper documentation before scheduling an appointment:

• Certificate of Medical Necessity (CMN) or prescription (Rx) for diabetic shoes/inserts (forms must be signed by an M.D. or D.O. only and is valid for 90 days)
• Current Medicare ID card
• Current Supplemental Insurance card
